The Care Plan is the country's first healthcare management company devoted to LGBTQ individuals and communities. It helps clients develop a personal health and aging guide working with individuals and families to restore clarity, comfort, and control.

Chicago based, Jackie Boyd founder and owner of "The Care Plan" has spent much of her professional career providing care management services.
